A fundamental requirement in every entity is to frame policies & processes and document these. Most organizations, do frame these in the initial days and publish them. But thereafter, only notify the changes from time to time, ignoring to update the originally published policies and processes.

Other day, newspapers carried headlines reporting that Police Rules were updated after 61 years!
